Observer Waiver and Confidentiality Acknowledgment

Required for anyone age 18 or older who wants to observe a Stratford EMS shift, such as students, healthcare partners and community leaders. The form explains the observer’s non-clinical role, the risks of riding in emergency vehicles, and the requirement to keep all patient information confidential. Complete and submit it before an observation shift can be scheduled. Shifts are scheduled based on operational needs.
