File of Life Program

About the Program
This program, provided by the Stratford Volunteer EMS Association, is intended to provide citizens of Stratford a method to store important medical information in their homes for access by emergency responders. The “File of Life” enables emergency responders (EMT’s, paramedics, firefighters and police officers) to obtain helpful information regarding a patient’s medical history. This is especially important if the patient is unconscious or is unable to speak to the emergency responders.

The “File of Life” includes a large plastic pouch, a magnet for the refrigerator door, and a medical information form. By completing the Medical Information Form (PDF) and attaching the magnet pouch on your refrigerator (or anywhere that responders can easily access it), you can provide vital life-saving information to the emergency responders.

A free “File of Life” can be picked up at the Stratford EMS – EMS Headquarters during normal business hours at:

Instructions

  1. Complete the Medical Information Form for each member of the household. Be sure to answer all pertinent questions that may be beneficial to responders.  Update the forms often.
  2. Place completed Medical Information Forms and a recent photocopy of a drivers license in the magnetic pouch.
  3. Place the File of Life on your refrigerator or anywhere that it is visible and can be easily accessed by responders.

It’s a good idea to review the information at least twice per year.
